There will be a “For Women Only Health Project” that Aims to Contribute to the over-all Reproductive Health of Women in the First District of Tarlac

The Carlos O. Cojuangco Foundation, Inc. (COCFI) has partnered with the Alfonso Yuchengco Foundation, Inc. (AY Foundation) for a worthy undertaking. Dubbed as “For Women Only Health Project” that aims to contribute to the over-all reproductive health of women in the First District of Tarlac by assisting women reproductive health diseases to get medical attention. Those who will qualify will be brought to a chosen private medical facility by AY Foundation within the district appropriate
medical intervention.

*TARGET CASES: *Non-Malignant gynecological disorders such as *myoma, ovarian new growth, ovarian cyst/polyp, uterine prolapse, phyllodes and others.*

*WHO CAN QUALIFY:*
1. Open to all women, 16-64 years old with combined monthly income of P40,000 and below;
2. Has been residing in the area for atleast 10 years.
3. Currently experiencing all or any of the ff. symptoms:
-prolonged menstrual bleeding
-with palpable mass protruding from the genital area
-with palpable mass in the lower quadrant of the abdomen
-ailment is not malignant and is treatable

*REQUIRED DOCUMENTS:*
ID, Birth Certificate, Certificate of Indigency either the MSWD and/or pastor/priest/barangay chairman, previous laboratory results (transvaginal/abdominal) if any, Certificate of non-filing of ITR.

Qualified patients must submit to District Office at *ICF College Bldg. Stall 3, Poblacion Norte, Paniqui, Tarlac, *the Patient Information Sheet (*see attached image) *together with the other documents NOT LATER THAN NOVEMBER 20,2019 to immediately schedule them for interview, medical screening and
medical procedures.
