The Civil Registry System (CRS) Tarlac Outlet of PSA has been inaugurated last September 23, 2022 in Tarlac City.

Residents of Tarlac City, other municipalities of Tarlac Province and neighboring provinces can now request certified copy issuance of civil registry documents ( Birth, Marriage and Death certificate) and certification ( Negative Certification, CRS Form No. 4 – CENOMAR and CRS Form No.5 – Advisory on Marriage) during office hours, Mondays to Fridays.

The following are the applied fees for each requested document:

P155 – Birth/Marriage/Death Certificates

P210- Certificate of No Marriage Record

The good thing is you can get your requested document within the day of your transaction.

The Tarlac CRS Outlet is now permanently located @ FP Corpuz Bldg. Mc Arthur Highway, Sto Cristo Tarlac City, beside PSA Tarlac Provincial Office
