– CERTIFICATE OF COMLPETION ( for Grade 10 Completers )
– PSA Birth certificate or Marriage Certificate
– Current Photos White Background with Collar ( 5 pcs. passport size )
– Long Brown Envelope with Clear Plastic Case
FOR ALS Graduate / COMPLETER
– ALS CERTIFICATE
– PSA Birth certificate or Marriage Certificate
– Current Photos White Background with Collar
( 5 pcs. passport size )

FOR COLLEGE LEVEL/TRANSFEREES / Undergraduate
– Official Transcript of Records
