How : Please Submit a PHOTOCOPY of the following
documents at the district office of Cong. Carlos O.
Cojuangco on or before FEBRUARY 01, 2018.

1. Duly accomplished application form which may
be DOWNLOADED from the DFA website,
www.dfa.gov.ph.

2. Birth Certificate (BC) in Security Paper (SECPA)
issued by the Philippine Statistics Authority or
Certified True Copy (CTC) of BC issued by the
Local Civil Registrar (LCR) and duly
authenticated by PSA.

3. Valid IDs and supporting documents to prove
identity (Valid IDs: COMELEC Voter’s ID, LTO
Driver’s License, PRC ID, UMID ID, PWD ID,
GSIS/SSS ID, Senior Citizen’s ID).

4. FOR MARRIED WOMEN : An additional
requirement of a PSA issued MARRIAGE
CONTRACT.
